Beginning October 1, 2009 new wallet identification cards from the Composite State Board of Medical Examiners will be mailed to new and renewing licensees. The new cards will come attached to a letter and are lightly laminated. You will peel off the card and fold it, then sign the back. Please watch your mail for a larger format envelope.
We believe we have improved the look and durability of your wallet license and hope you will like the change. Should you misplace your card or need duplicate copies for any reason, you may order these at the online renewal site for a cost of $10 per card.
